Eligibility Form Requirements


Below are the necessary steps/forms that Madison College Student-Athletes need to complete to practice and compete in intercollegiate athletics at Madison College.
(Only complete these forms/steps if you have been instructed to by your coach)

Step 1: Confirm all final/official High School transcripts & additional college transcripts are on file at the college
  • Transcripts must have a graduation date indicated on the transcript.
  • Make sure you get credit for prior learning and AP tests
  • Transcripts may be sent to the college, mailed, or delivered in person.
  • Official college transcripts must be provided for those who have attended and taken classes at any other college or university.
  • For those with college transcripts, we suggest that you send the transcripts via parchment to the college and order 3-4 official copies for yourself.
Step 2: Complete the Madison College Athletics Eligibility Form

Step 3: Complete the NJCAA Student-Athlete Registry Form - ONLY necessary for first-year student-athletes
  • Make sure to use your Madison College email address when completing the registry form
Step 4: Schedule a Physical & Get a digital copy of the completed Physical form (PDF) with all necessary signatures
  • Student-athletes are encouraged to schedule a physical exam with their primary care provider. If you cannot, then another option is to schedule a physical at the Madison College GHC Community Clinic.
  • Please print out the form and have it completed before your doctor's appointment. After the appointment, the doctor will give you the last page of the four-page physical form.
  • You will need to upload the physical form in Step 5. If you have a paper copy of the physical form, you can digitize the form by scanning the form into a PDF, request a digital copy from your doctor, take a photo of the form, etc.
Step 5: Complete the Physical & Insurance Form
  • You will need to upload the digital-physical form, so have it ready to upload.
  • This is a good form to complete with your parents available as many questions pertain to insurance.
Step 6: Complete the Madison College Athletics Release Form

Step 7: Complete the sport-specific Student-Athlete Questionnaire

Esports Specific Eligibility Requirements
